The Uncontested Divorce DIY (Do-It-Yourself) Form Program is a free computer program for people who do not have an attorney. The program guides the user through the program, by asking a series of questions.  At the end of the program, the user will receive the forms they need to file with the court along with step-by-step instructions.  There is an option to print the instructions in Spanish as well as English. 

The Queens Supreme Court in Jamaica, NY, has a dedicated Uncontested Divorce DIY Form Program terminal room, with four computer stations on the second floor.  To ensure privacy, the papers are printed in the Matrimonial Clerk’s office located on the first floor, in Room 140.  You may bring a thumb drive, to save a copy of the paperwork prepared by the DIY Form program.

For those unable to visit the courthouse to prepare their forms, the DIY Form programs may be accessed anywhere you have access to a computer, the internet, a printer, Microsoft Word, and Adobe Flash.  The papers must be filed in person at the courthouse.  Visit the DIY Form Menu, to see the many DIY Form programs offered by the NYS Unified Court System.  For information about the law and court procedure, visit CourtHelp.
