- use the new online ADA Request for Accommodation Pilot form for Suffolk County, 10th Judicial District.

- for 3rd District Court send an email to the District Executive at ADASUFFOLK@nycourts.gov. Please put "ADA Accommodation Request" in the subject line.

- for Commissioner of Jurors send an email to the Juror personnel at suffolkjury@nycourts.gov. Please put "ADA Accommodation Request" and the "Juror Index Number" in the subject line.

ELEVATORS: Facility does not have elevators.
RESTROOMS: Available on the 1st floor.
PARKING: Public parking on-site or adjacent to the facility.
ADA ENTRANCES: Main Entrance: Entrance is accessed by a ramp and Court Officers are also available to assist, if needed.
